I understand that in order to attend Detroit Mar Thoma Church Holy communion service, I must follow DMTC’s health and safety compliance with Michigan state requirements for churches. This includes not attending a service if I or anyone in my household feels ill or shows symptoms of sickness prior to Sunday. This includes, but is not limited to: high fever (100.4 F or greater), coughing, labored breathing or fatigue. I also understand that if I’ve been exposed to or are currently being tested for COVID-19 , I must stay home per state regulations. Finally, I understand that I must bring a mask and wear it the entire time I’m on the DMTC campus. By attending Detroit Mar Thoma Church in-person Holy Communion Service, I agree to abide by the procedures established by the church to protect the attendees and I voluntarily assume the risk that my family and/or I may be exposed to or infected by COVID-19 either at the church or in the church worship service. I agree to assume all the risks of attendance and participation for my family and me, and I waive any liability against the church and any other parties.
